Purchase Orders

All purchase orders are reviewed on a case by case basis, and must accompany all verification documents (see below). Please Note: All Purchase Orders are subject to 10 day payment terms.

Customers wishing to place orders using a Purchase Order can do so by following these steps:
  1. Contact us by phone to place the purchase order: 1-877-930-5690
  2. Fax the following paperwork to us at 419-335-2240:
    • Credit Application
      • Setup a charge account by filling the following form and faxing it to 419-335-2240.
      • Click here for more information including terms regarding setting up a charge account.
    • Bank References
    • Vendor References
    • Federal Tax ID
    • Purchase Order request on company letterhead
  3. We will contact you if and when the credit application has been processed and you are approved. Purchase Orders typically take 5 business days to process
